The 'Add Note' tab is available in the issue editing mode, and not — while you creating a new issue.

Private note

When writing a note and marking it private, the note becomes invisible to all users except the author of the object you are writing the private note to.

Also, you can add email as a note. Drop an email on the issue if you want to add it as a note.
